Stillwater Assisted Living and Skilled Nursing Community is seeking a Wellness Director (RN or LPN).

The primary responsibility is to provide direct nursing care to the residents and supervise the day to day operations of the nursing team: Nurses, Med Techs and Personal Care Assistants. Such supervision must be in accordance with current federal, state and local standards, guidelines and regulations that govern our facility to ensure the highest degree of quality care.

• Ensure that all nursing staff is in compliance with their respective job descriptions.
• Ensure that department personnel, residents and visitors follow the departments established policies and procedures at all times.
• Ensure that all nursing service personnel comply with the procedures set forth in the Policy and Procedure Manual. Follow established infection control techniques and safety regulations in the use of equipment and supplies.
• Supervise staff, including orientation of new staff, assignments, training, mentoring and evaluations.
• Meet with nursing personnel on a regular basis to assist in identifying and correcting problem areas, and/or improve services.
• Complete assessments on potential residents, complete health assessment within one week of move in,
• Assist with the creation of a service plan, changes as needed and annual assessments.
• Maintain communication with residents and their families.

Qualifications: Must possess a valid nursing license in the state of Rhode Island.
